This role involves integrating an Ability United Member into your family unit and community, providing a personalized and caring alternative to traditional residential care settings. You will play a pivotal role in enhancing the quality of life for the Member you host, supporting their independence, and fostering their involvement in community activities. Key Responsibilities: Provide a safe, welcoming, and inclusive living environment for the Member. Assist with daily living activities, including but not limited to, personal care, medication management, and mobility support. Coordinate and provide transportation for appointments, social activities, and other necessary outings. Plan and prepare meals considering dietary needs and preferences. Facilitate and encourage the Members engagement with the community. Collaborate with care teams to implement personalized care plans, attending to the physical, emotional, and social needs of the Member. Maintain regular communication with the agency, updating them on the Members progress and any concerns or needs that may arise. Conduct and maintain daily documentation of the Members health, activities, and overall well-being, ensuring accurate records are maintained. Requirements: A compassionate, patient, and understanding demeanor. Ability to provide a stable and nurturing home environment. Strong interpersonal and communication skills. Ability to work flexibly and respond to changing needs. Commitment to promoting independence and integration in the community. Must pass a background check and meet agency home safety standards. |
