Coordinate community supports such as therapists, physicians, schools, etc. are in place and utilized to implement the Treatment Plan. Document progress in the monthly review and submit to TFACTS by the 15th of each month.oMaintain lines of communication with the youths guardian/parents, FSW, therapists, teachers, doctors, etc., and keep all necessary parties informed of the youths progress.oEstablish lines of communication with FSW, youth, and family members to develop and distribute the welcome packet, initial intake assessment, crisis plan, and phone call visitation schedule, and face to face contact note within 72 hours of admission.oEnsure the family participation in therapy and participation in goals to achieve reunification and permanency. Provide documentation of family involvement and efforts to include family in all aspects of the youths care.oAssure that all aspects of the youth(s) served records (hard copy and electronic) are current and conform to all regulations and standards.oAssure that all required/recommended medical and therapy appointments are completed and that any follow-up appointments are scheduled and documented in the electronic record.oAssure that all incidents are reported according to TFACTS as required. Assist with internal and external investigation requests, including documentation requests and coordinating staff interviews.oEnsure that supervision visits are completed and documented. This includes four (4) unannounced visits in each home per month, including an overnight visit. Also, follow up on identified issues from Quality Assurance visits. Supervise employees, including completion of evaluations and disciplinary actions. Make recommendations regarding hiring and termination of employees.oThe Case Manager is on call and is responsible for maintaining a cell phone 24/7 to ensure accessibility. The Case Manager is required to respond to all calls within 30 minutes from Staff /Management regarding issues, aggression, physical violence, property damage due to behaviors, and incidents requiring law enforcement or emergency medical intervention.oMay give telephonic guidance; however, personal attendance to a crisis may be required at any time. Assist home staff in managing incidents and emergencies. Follow up to ensure that incidents are reported according to policies and procedures in a prompt and timely manner.oAssist with transitions and admissions to all programs that Encare provides.oEncourage customer satisfaction through regular contact with youth served, conservators, families, and external stakeholders.oEnsure that scheduling and tracking of all medical and therapy appointments, including follow-up via persons served records, is completed. Assist with maintaining medical and therapy grids and follow-up, as required.oEnsure youths served, nursing, and team members are informed of all appointments or assessments for the youth served. All appointments and follow-up should be noted and maintained within the persons served electronic record or via hard copy when necessary. Assist with appointments when appropriate.oProvide documentation, as needed, to external entities to ensure supports for youth served.oDocument meeting outcomes and review each Treatment Plans or drafts for errors for needed revisions.
