My name is Candidate's Name  and I am currently the Assistant Warden of Programs for Stateville Correctional Center in Crest Hill, Illinois. As a Black man growing up in the inner-city of Chicago, I know first-hand of the negative impact of mass incarceration and recidivism on our community. I have dedicated the past 23 years of my career to making a positive impact by being a Certified Substance Abuse Counselor (CADC), MISA specialist, and leader within IDOC.Prior to my role at Stateville, I acted as the lead counselor at Sheridan Correctional Center, and am proud to say that I personally spearheaded the implementation of new programing within this facility, to include the Young Mens Aggression Management Group and Another Chance programming. As a result of my teams efforts, Sheridan boasted a significantly lower recidivism rate than IDOC as a whole. I am very excited about the opportunity and challenge of implementing similar success within my community via the Greater New Mount Eagle Baptist Church.Mass incarceration and recidivism are factors that have a significant negative impact on public health and safety of the people living within the communities of the inner-city of Chicago, particularly the citys Southside. While IDOC has taken steps to help lower recidivism rates by increasing programing and re-entry services within the past couple of years, poor connection to community-based resources upon release is often a contributing factor to relapse and subsequent recidivism within the criminal justice system. Securing the resources available through this grant opportunity will permit the Greater New Mount Eagle Baptist Church to improve the current system of community linkage and aftercare services for individuals in custody paroling/discharging from prison or jail. In addition, resources will be allocated in order to prevent at-risk youth from being involved within the legal system in the first place.Summary of Program Design and ServicesIn order to help reduce recidivism and prevent at-risk youth from being involved with the legal system, the following evidence-based services will be provided via Greater New Mount Eagle Baptist Church by Candidate's Name , CADC, and his skilled clinical team: Comprehensive substance abuse counseling and group therapy for adults and adolescents suffering from drug/alcohol use disorders. Community Re-integration and Re-entry services for those transitioning from prison/jail into the community. Peer mentorship and support groups for at-risk youth and individuals recently involved within the legal system. Support groups and empirically-based interventions for children and adolescents whose parents are currently or recently incarcerated. Linkage and support services, to include, assisting individuals obtain State IDs and enrollment in other entitlement programs, such as SSI and Medicaid. Individualized case management services in order to connect those needing mental health services and a higher level-of-care (e.g., inpatient substance abuse treatment) to local providers and assistance with housing placement within the community.
