Final Pay and PTO InformationWhen an employee terminates, the final paycheck is issued in accordance with applicable state law. Compensation for hours worked between 11/12/2023 and 11/17/2023 will be paid out on your last regular paycheck, dated 11/30/2023. This will be paid in the traditional method and will be available for viewing through ADP at https://workforcenow.adp.com/public/index.htm. Gain Share / Bonus Incentive PlanPlan participants who quit, leave by mutual agreement, or are terminated prior to the end of the quarter are ineligible for plan payout participation. To be eligible for plan payout, a plan participant needs to be an employee in good standing both on the last day of the period, quarter, and/or fiscal year and be employed on the subsequent dates of the plan payout. You are not eligible for a bonus payout. Benefit Coverage InformationYour Health (Medical, Dental, Vision) coverage, if you had elected coverage, will end on the last day of the month. You can continue your coverage for 18 months via COBRA. To access COBRA information please go to: https://mybenefits.wageworks.com/Register.aspx All other lines of coverage end on your last day of employment. 401(k) InformationIf currently enrolled in 401(k), you will have the option to either 1) leave your money in the company- sponsored retirement plan, 2) roll your money into a new account, or 3) cash out (taxes may apply). Company PropertyPlease make sure you have gathered all personal items from your desk, workstation, locker, etc. If you have any company items (keys, tools, toolbox, ID Badge, etc.) please return those to your Manager. All Uniforms and Lab Coats should be returned to the bin in the locker room prior to your last day. If you have any at home, please make arrangements to return these during regular business hours, within 48 hours of your termination date or you will be charged for missing items.
