Case manager received referrals from doctors  offices and DCF letting the department know that there were clients that needed assistance that were pregnant or had children living at home that needed assistance up to the age of 3 years old. Human Services Counselor goes out and discusses what the needs are of the client and see if they will agree with the assessment. If they do, then we give them the services needed so that they will be able to stay at home and not have to be put in a facility.

CM calls the client and schedules a time and date to see the client within 72 hours (about 3 days) to get services given to the client. CM goes out and discusses what the needs are of the client and see if they will agree with the assessment. If they do, then we give them the services needed so that they will be able to stay at home and not have to be put in a facility.

 As a Referral Coordinator, I had to look up the provider for the member, then key in the code for Office Administration, Outpatient or Ambulatory; call member and make sure you were speaking to the correct person because of the HIPPA laws. We would have to authorize the member to have the procedure or visit with the physician, rehab therapy, x-rays.

In the notes we would have to put in the CPT codes for diagnosis, referral, treatment of new and old patients; make note of the patient call after call; summarize the progress notes/demographics, ambulatory for surgery with provider and facility listed. Create digital images of paper documents using multiple types of scanners. Making sure that the documents were processed through quality control.
