| | Click here or scroll down to respond to this candidate1 P a g eSOUTH CAROLINA OFFICE OF RURAL HEALTHTITLE: Perinatal Social Worker/Reproductive Health Specialist REPORTING RELATIONSHIP: Director of Social WorkPOSITION SUMMARY:Provide planning, social work, and reproductive health services to Family Solutions of the Low Country (FSLC) maternal, adolescent and infant clients. Provide counseling, coordination, and education. Coordinate and implement activities of (FSLC) to assure goals, objectives and strategies of the program are met.QUALIFICATIONS:Education and/or Experience: Masters or Bachelors degree in social work. Experience in working with families, culturally diverse communities and community agencies. Competencies: decision-making, problem solving, organization, planning & organizing, communication, coaching, teamwork and adaptability.DUTIES: Work closely with community health workers and assist with work performance. Plan/Coordinate/facilitate the Multidisciplinary Team Meetings. Improve the health status for women of childbearing age (ages 10-44) Provide social work services and psychosocial assessments to FSLC target population/families through home visiting. Provide individual, crisis intervention counseling, short-term behavior treatment, along with follow up services, as they relate to improving pregnancy outcomes and general health care. Develop participants safety plan. Provide curriculum education relevant to prenatal or postpartum period Monitor service utilization with clients for adequacy of health care. Develop and implement service plan. Participate in ongoing training to increase knowledge about Maternal Child Health. Provide timely written reports and drafts for use in developing the annual grant application, progress report, data reports and other reports required by either to Federal Bureau for Maternal and Child Health or the SC Office of Rural Health. Assist Program Director in building and/or strengthening partnerships. In partnership with Choose Well, reduce unintended pregnancies and increase usage of LARCs Case manage up to 15 women identified as being noncompliant with birth control, those who are medically high risk of which subsequent pregnancies could result in a poor outcome, and those identified as having a short interconceptional period. Assist women in setting short and long term reproductive health goals with emphasis on social needs, barriers to care, family planning, contraception, and understanding how pregnancy occurs. Maintain a high level of health expertise relevant to family planning and reproductive 2 P a g ehealth and share up-to-date information with FSLC staff and partners via trainings and/or during MDT meetings. Attend trainings to increase knowledge and enhance service delivery skills |
