Position: LPN(Licensed Practical Nurse)

Location: York, PA 17402

Four shifts available : (10:45PM - 7:15AM) or (6:45AM- 12:15PM) or (3PM - 11:30PM) or (11PM - 7:30AM)

Position Overview:We are seeking a compassionate and dedicated and Licensed Practical Nurse (LPN) to join our nursing facility team. The LPN will provide high-quality care to residents requiring post-hospital short-term rehabilitation and those in need of long-term care. This role is ideal for nursing professionals who enjoy diverse clinical settings and are committed to enhancing the quality of life for patients.

Key Responsibilities:

Patient Care: Deliver direct nursing care to residents, including administering medications, monitoring vital signs, and assisting with daily living activities.
Rehabilitation Support: Assist in the implementation of individualized care plans for residents in post-hospital rehabilitation, collaborating with physical and occupational therapists.
Long-Term Care Support: Provide compassionate care to long-term residents, fostering a supportive and nurturing environment.
Documentation: Maintain accurate and timely documentation of patient assessments, care plans, and progress notes in compliance with facility policies and regulations.
Collaboration: Work closely with a multidisciplinary team, including RNs, physicians, therapists, and support staff, to ensure comprehensive patient care.
Patient Education: Educate residents and their families on health management, medication, and rehabilitation processes.
Emergency Response: Respond effectively to medical emergencies and provide basic life support as needed.
Qualifications:

Licensure: Current and valid LPN license in State or eligible for licensure by endorsement.

Pay: $32.00 - $34.00 per hour
