Job Description
Fast paced outpatient treatment center seeking a qualified professional who is passionate about the field of drug and alcohol abuse and mental health. Case Manager will work closely with client, clients family and staff to assist client in achieving goals through individual sessions, group facilitation, treatment planning, family involvement, linking client with community resources, discharge and after care planning.
Duties & Responsibilities:
- Drug and alcohol counseling on an individual basis
- Include families in treatment as needed/ recommended
- Meets with each client on their caseload to check progress in treatment including but not limited to living situation, relationships, employment, education, need/desire to extend treatment, discharge plan, etc.
- Make sure that client meets minimum number of program hours required.
- Facilitate group sessions on a variety of specialty topics, ie relapse prevention, life skills, mindfulness, aftercare planning, etc.
- Contribute to a healthy work environment
- Actively participate in meetings and team-building activities.
- Ensure a community environment is maintained
- Ensure health and safety regulations are upheld to our organizational standards
- Act as a client advocate and assist clients with problem solving
- Provide crisis intervention and emotional support to clients when appropriate
- Maintain contact with each client as required by contract and program guidelines
- Maintain accurate & updated information in each client's chart and EHR system, to our organization and JCAHO/CARF standards
- Facilitates in weekly programs and services team meetings.
- Supervise client treatment progress, charting and discharge planning.
Qualifications:
- CAADE, CAADAC or NAADAC Certified Drug Counselor or certification in related field.
- 1+ years’ work experience in an alcohol/drug dependency treatment programs.
- Working knowledge of CARF and/or JCAHO/TJC accreditation standards
- Knowledge of the 12-step, Smart Recovery and other treatment models
- Knowledge of HIPPA
- Good documentation for medical necessity and as required by insurance
- Excellent communication, problem solving, teamwork, motivational and decision-making skills.
- Proven skills in substance abuse crisis intervention.
- Sensitivity and ability to work effectively with a diverse population (racial, ethnic, national origin, religious, gender, sexual orientation)
- Strong professional references attesting to high degree of customer service, punctuality & professionalism.
- Must have Valid California Driver’s License, reliable transportation and a clean DMV record
Job Type: Full-time
Salary: $18.00 to $25.00 /hour
