Unicare Adult Day Care Health Center seeks Geriatric Social Worker.

Master's degree in Social Work or closely related field required.

Job duties:
1. Interview and screen all applicants/referrals to determine appropriateness for a full assessment process and participation in the adult day health care program;
2. Develop a plan of care for elderly clients by identifying their goals and concerns;
3. Conduct initial assessments, quarterly and six-month reassessments for each participant, which includes evaluating and reviewing progress made towards identified goals and updating plan of care as needed;
4. Provide various care management services to participants in order to obtain needed services such as IHSS and Access;
5. Provide referrals as needed to available community resources such as housing referrals and access to social welfare programs;
6. Provide a treatment program to clients including individual mental health counseling services and emotional support groups.

Salary: $58,614/year
