This is an HSCRC funded position through 2025.
Position Summary
Participate in initiatives to improve diabetes care delivery, including use of diabetes technologies and remote patient monitor program
Oversee planning, coordination, and facilitation of regular meetings, workgroups, and learning collaboratives to ensure goals are met. This includes, but is not limited to, leadership of: DPEP; Division of Endocrinology, Diabetes & Metabolism; participating hospitals; participating ambulatory practices; academic training programs; access services; nursing leadership; OPH; Armstrong Institute Diabetes Clinical Community.
With staff, conduct outreach and recruitment efforts to community organizations and agencies to participate in initiatives and in recruitment of clinicians and/or admin staff to support DSMT
Establish and maintain strong relationships with key stakeholders (hospital leaders, population health leaders, diabetes clinical leaders, case management, clinicians, informatics leaders, etc.)
Establish individualized workplans and metrics with stakeholders that align with overall program goals (i.e. patient participation in DSMT and/or improved diabetes clinical outcomes, such as quality measures, PQI-93, etc.)
In partnership with the DSMT Program Director, facilitate oversight of quality and efficiency of CDCESs and inpatient diabetes nurse navigators.
Work with marketing teams to promote DSMT service to clinicians and general public
Facilitate sharing of best practices across the multiple and varied care delivery sites of DMST

No weekend work required
Education/Experience
NP, PA, or MD degree
Three (3) plus years of direct patient clinical experience in the field of diabetes
