Service Support Facilitator (Remote)

Who we need:
The QRTP Service Support Facilitator answers calls from community members who are seeking support to ensure their child is receiving the proper level of behavioral health services. The service support facilitator assesses the child/family via the telephone and/or face to face to gain information to determine which level of care best meets the childs needs. Clinical interviewing and assessment skills required.

What youll do:
1) Develop process, conduct and appropriately document member assessments within prescribed timelines
2) Develop/enter assessment data into the a web system
3) Develop/participate in inter-rater activities to ensure consistency in the administration of the assessment instruments and adherence to assessment protocols
4) Develop and conduct training with provider agencies and other stakeholders as needed/assigned
6) Complete special projects as assigned
7) Other duties as assigned.
What youll need:
Required Qualifications
- Bachelors Degree in a related healthcare field
Knowledge, Skills, Abilities
- Applicant must possess knowledge of person-centered planning fundamentals. Applicant must have a familiarity with computer applications and be able to navigate through multiple screens and have the ability to maintain confidentially standards and HIPAA compliance when evaluating all pertinent issues.
Experience
- 2+ years of experience working with children receiving mental health service and possess knowledge of Child Welfare, Behavioral Health and Medicaid policies and procedures.
