We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. Responsibilities

• Implement therapeutic exercises and modalities to promote healing and recovery.


• Educate patients and their families on treatment plans, exercises, and lifestyle modifications.


• Monitor patient progress and adjust treatment plans as necessary.


• Collaborate with other healthcare professionals to ensure comprehensive patient care.


• Participate in discharge planning to facilitate smooth transitions for patients from inpatient to outpatient care.


• Maintain accurate documentation of patient evaluations, treatments, and progress notes.

Requirements

• Valid Physical Therapist license in the state of practice.


• Experience in home health, hospital settings, orthopedics, or acute care preferred.


• Strong knowledge of anatomy, physiology, and rehabilitation systems.


• Ability to create effective discharge planning strategies for patients transitioning from inpatient care.


• Excellent communication skills for interacting with patients, families, and healthcare teams.

• Ability to work independently as well as part of a collaborative team.
