Summary/Objective: The care coordinator assists TeleVine Therapy in accomplishing its mission of providing quality therapy with caring services to all populations in order to enhance their communication, swallowing, cognition, and overall lives with attaining great results. The patient services coordinator supports our families by serving as a receptionist.
Essential Functions:
*Schedule initial evaluations, reevaluations, and therapy sessions for patients receiving Speech-Language therapy services
*Coordinates and communicates therapist personal leave/sick leave scheduling and makeup appointment/PRN scheduling to families to minimize missed therapy sessions for our patients.
*Confirms therapy appointments with families the day before through text messaging/phone call/email.
* Ensures the accurate and complete gathering of all patient data required at the initiation of therapy services and any information that is requested by a therapist to complete a plan of care.
*Checks in patients in a timely and prompt manner.
* Performs various related clerical duties supporting department operations including, but not limited to, answering telephones maintaining files, scanning, keeping inventory of supplies, and informing management of all activities, needs, and problems.
*Maintains assigned Key Performance Indexes (KPIs) and enters data daily to ensure accurate data/clinic management.
*Completes “remove red” appointments in a timely manner to avoid non-payment of services.
* Guide patients through this sometimes difficult new patient process by kindly talking them through the steps and answering their questions and concerns.
*Coordinate with office staff and therapists for timely care of patients.
*Cross-train and help as needed in other departments (i.e. scheduling, medical records, etc).
*Participate in various projects and/or meetings, and complete other tasks as assigned by management.
*Maintains patient confidence and protects clinic operations by keeping information confidential.
*Maintains safe and clean working environment by complying with procedures, rules, and regulations.
* Protects patients and employees by adhering to infection-control policies and protocols.
* Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs.
*Provides translation services as needed for therapists and families.
*Completes any additional job duties as assigned by any member of management.

Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, filing cabinets and fax machines.
Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand; walk; reach; bend/stoop; squat throughout the day. The employee should be able to lift up to 25 pounds.
Recommended Education and Experience
1. High School Diploma or General Education Degree (GED) is required for this role, as well as 1-2 years of experience in medical or related field.
2. Must have good communication and interactive skills to work with patients, physicians, and peers.
3. Ability to multi-task successfully and excellent organizational skills.
4. Must have the ability to work with computers and quickly acquire an understanding of changes in processes.
