RESPONSIBILITIES
- Complete initial comprehensive assessments and periodic reassessments to determine the services and supports required by clients to prevent unnecessary institutionalization
- Research the availability of non-Medicaid resources needed by clients
- Act as the client’s advocate as it relates to insurance coverage and financial assistance
- Assist clients in gaining access to available Medicaid State Plan services
- Locate appropriate home and community-based settings and assist with negotiation of rental agreements when needed
- Maintain the client’s record through detailed documentation
- Monitor to assure provision and quality of services
QUALIFICATIONS
- Bachelor’s degree in Social Work or related field
- Licensed as a Social Service Worker (SSW), Certified Social Worker (CSW) or Licensed Clinical Social Worker (LCSW)
- 1-3 years of experience, preferably in case management or related area
SKILLS
- Knowledgeable about Utah Medicare/Medicaid systems and community resources
- Comfortable working with disabled adults
- Proficiency in MS Office with expertise in Microsoft Word and Excel
- Strong written and verbal communication skills
- Able to work independently
OTHER REQUIREMENTS
- Willing to travel to visit clients located in Salt Lake County and Utah County
- Access to reliable transportation
- Available to attend weekly staff meetings
