We’re looking for an individual with a Master’s degree in Social Work and long-term care experience to join our new team in Hawaii. As the Field Service Coordinator, you would manage the care of members in their homes or other community settings, focusing on disease management and keeping members healthy and independent. Key responsibilities of the Field Service Coordinator include:

• Visiting members in their homes, assisted living facilities or other settings
• Delivering a holistic approach that results in effective, consistent, collaborative and coordinated care based on member needs
• Conducting psychosocial assessments with members
• Meeting the needs of members through identification of early risk factors
• Focusing on disease management and decreasing unnecessary hospitalizations
• Creating a care plan
• Collaborating with the primary care physician to deliver and coordinate necessary services
• Building relationships with members and their families

• A Master’s degree in Social Work
• Clinical experience
• Long-term care experience
• Excellent verbal and written communication skills 

• Licensure in Social Work
• Bilingual (Chinese, Filipino, Korean, Samoan or Vietnamese) skills
• A background in case management and/or behavioral health
• Familiarity with Medicaid or Medicare
• Experience with special needs pediatrics
• Familiarity with electronic documentation
• Proficiency in Microsoft Excel
